![]() | Responsibilities: Obtains complete medical history and physical data on patients.Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures as needed.Performs a complete physical exam and records findings. Collaborates with providers in managing acute and long-term medical needs of patients.Orders appropriate laboratory and diagnostic procedures.Synthesizes data to determine diagnosis and therapeutic plan utilizing principles of prevention.Administers medications and injections. Sutures minor lacerations.Interviews and advises patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.Instructs patients and family regarding medications and treatment instructions. Provides patient education.Maintains and reviews patient records, charts, and other pertinent information. Posts tests and examination results. Notes need to be co-signed by physician.Triages patient telephone calls and provides consultation.Manages medical and surgical emergencies.Provides monitoring and continuity of care between visits.Have a working knowledge of ICD9, CPT and HCPCS coding and managed care, and be available for training as necessary.Attends required meetings and participates in committees/pilot projects as requested.Participates in professional development activities and maintains professional affiliations.Follows the Legacy guidelines related to the Health Insurance Portability and Accountability Act (HIPAA), designed to prevent or detect unauthorized disclosure of Protected Health Information (PHI).Maintains strict confidentiality.Uses oral and written communication skills to effectively convey ideas in a clear, positive manner that is consistent with the LEGACYMission.Maintains established LegacyParticipates in the Performance Improvement Program Requirements: Graduate of an accredited school of nursing.Successful completion of an approved Nurse Practitioner program.Three years professional nursing experience in a clinic setting.Must be fluent in English and Spanish.Possession of a valid State Registered Nurse license and Advanced Practice license certified by an accredited certification agency recognized by the state board of nursing.Current CPR certification. |
